It would be nice if he would share on how he created some of these forms. Cause I would've put them on my website with him as the owner of them.

Those placefiles were most likely created using PHP.

Most people who write those from scratch aren't normally willing to put them into the public domain for just anyone to grab and use. There have been several instances of this happening over the years from a few folks, who made placefiles for their own use and then offered it to their friends for free and found themselves without an ISP, because they violated their ISP's TOS on hosting a personal server and going far, far over their monthly data usage. One that comes to mind had Comcast as their ISP provider being shutdown by Comcast because of data usage and going several hundred gigabytes over their data cap and had a to pay Comcast for that overage which was at least $2,000. By the way, that over $2,000 was for 1 month, not over several months.

Something to keep in mind if you are looking into doing this.
